Prior to initiating a new software request, please have the software vendor complete the “Software Evaluation Survey” using the link OR the QR code below.
Once the “Software Evaluation Survey” has been submitted, “New Software Requests” can be submitted for consideration via Incident IQ:

1. Log into Incident IQ
2. Select “New Request”
3. Select the “Communications Dept. and Other Requests” Tile
4. Select the “Technology Evaluation” Tile
5. Complete and submit the ticket to initiate your New Technology Evaluation Request
